Admission to the Augusta University is contingent upon meeting all standards and requirements of undergraduate admission and upon passing a criminal background check prior to matriculation at Augusta University AND into the BSN Program. I hereby agree that if I am accepted into and successfully complete all requirements of the Freshman and Sophomore years as a pre-nursing student and desire to enter the BSN Program, I will complete my professional nursing school education on the Augusta Health Sciences Campus at Augusta University. In submitting this application, I hereby certify that all statements herein are true, correct and complete, that I have not omitted any information requested, and that I have read and I understand the requirements of the Augusta University Nursing Scholars Program. “I understand that any material false statement made knowingly and willfully by me on my application, or any documents attached hereto may, in accordance with O.C.G.A. 16-10-71, which provides that upon conviction, a person who knowingly commits the offense of false swearing shall be punished by a fine of not more than $1,000 or by imprisonment for not less than one nor more than five years, or both, subject me to prosecution in a court of law. Additionally, I further understand that any such false statement may subject me to immediate dismissal from the institution.
